• I hereby certify that I am the legal owner/carer of my "Pet", and am of legal age of (21 years and above) to authorise care and treatment of the animal at BEECROFT ANIMAL SPECIALIST & EMERGENCY HOSPITAL, or Beecroft.
• I hereby authorise the veterinarian to examine, prescribe medications and treat my pet.
• I take full responsibility for all charges incurred in the care of my pet.
• I hereby certify that details on this record is accurate and up-to-date date.
• I permit Beecroft to take photographs and videos of my pet(s) without compensation to use any images. I understand that these materials may be used for the hospital social media and website to maintain its business or services, for educational purposes, and using pet's first name only, if at all. I hereby release & discharge Beecroft from any claims arising out of the use of photos and videos. This authorisation is continuous and I can withdraw it by writing into (hello@beecroft.com.sg)
• I acknowledge that there are inherent risks associated with the diagnosis and treatment of animals. Complications or adverse outcomes can range from minor to fatal. The probability of a complication is typically related to a patient’s health status or may be procedure-specific, however even minor medical procedures and diagnostics carry risk. The team at Beecroft endeavour to perform all diagnostics and treatments in a safe and efficient manner to minimise risks and optimise outcomes.
